This package is provided to:
• Children/young people who may or may not have worked with the SLT before.
This involves:
1. Information gathering from parents and, with parental permission, from school/nursery. Should any additional phone liaison with other professionals is required, with parental permission, this will be conducted at no additional cost.
2. Working with the child and conducting specific informal assessments tasks. This is to obtain a picture of the child's functional language and communication ability, strengths and needs.
3. Conducting formal assessments with the child as necessary and appropriate. This may involve using standardised tests to obtain a quantitative measure of specific aspects of the child's speech and language skills, to enable a more in-depth profile of the child's strengths and needs.
4. A classroom observation to provide a fuller picture of the child's speech-language and communication needs within his educational setting.
5. Feedback to school and family on the assessment findings and their implication to therapy and longer-term needs.
6. A detailed SLT report to contribute towards an EHC Plan, with specific recommendations for support & provision. The report will be completed in a format consistent with that which is adopted for EHCPs by the local authority. The draft report will be released for parental verification only after full payment has been received. Parental consent must be confirmed before reports are shared with school. Parents will receive a copy of the report as a matter of course.
Full payment for the whole package must be received no less than 1 week before the first session of this package takes place.
This package does not include attendance at EHCP review meetings, which will be charged separately at Consultation/Meeting rate.
